Player Evaluation Forms provide the best opportunity for Medway Youth Soccer to maintain equality in skill level amongst the teams.  It is imperative that each player be evaluated on a scale uniform to all players in the division.

Intramural coaches will need to fill out two forms. One form must be filled out for the U-8 players on their team.  The second form for the U-9 players, moving to the travel program next season.

Process for completing Player Evaluations

1. If you want to complete the Player Evaluations electronically ...
   1. Download the Microsoft Word version of the player evaluation form.
   2. Using Microsoft Word, complete the player evaluations.
   3. Send the completed player evaluation form as an attachment electronically, via e-mail to the following depending on your league"
      • Instructional – pakelley@comcast.net
      • Micro – pakelley@comcast.net
      • Intramural (U-8 players) – pakelley@comcast.net
      • Girls U-9 & up – cdroeske@yahoo.com

2. If you want to complete the Player Evaluations manually ...
   1. Download the PDF version of the player evaluation form.
   2. Fill out the player evaluation form.
   3. Send completed forms to the following address:

All U-11 evaluation forms are due by May 7^th

All other ages group evaluations are due no later than June 1^st
